            <paragraph>Cefdinir for oral suspension, USP contains the active ingredient cefdinir monohydrate, USP, an extended-spectrum, semisynthetic cephalosporin, for oral administration. Chemically, cefdinir monohydrate, USP is (6<content styleCode="italics">R</content>,7<content styleCode="italics">R</content>)-7-[[(2Z)-(2-amino-4-thiazolyl)(hydroxyimino)acetyl]amino]-3-ethenyl-8-oxo-5-thia-1-azabicyclo[4.2.0]oct-2-ene-2-carboxylic acid monohydrate. Cefdinir monohydrate, USP is a white to light yellow crystalline powder. Its solubility is 19.56 mg/mL in 0.1 M pH 7 phosphate buffer.                 </paragraph>
                <paragraph>Maximal plasma cefdinir concentrations occur 2 to 4 hours postdose following capsule or suspension administration. Plasma cefdinir concentrations increase with dose, but the increases are less than dose-proportional from 300 mg (7 mg/kg) to 600 mg (14 mg/kg). Following administration of suspension to healthy adults, cefdinir bioavailability is 120% relative to capsules. Estimated bioavailability of cefdinir suspension is 25%. Cefdinir oral suspension of 250 mg/5 mL strength was shown to be bioequivalent to the 125 mg/5 mL strength in healthy adults under fasting conditions.</paragraph>

                  <content styleCode="italics">Effect of Food</content>
                </paragraph>
                <paragraph>In adults given the 250 mg/5 mL oral suspension with a high-fat meal, the C<sub>max</sub> and AUC of cefdinir are reduced by 44% and 33%, respectively. The magnitude of these reductions is not likely to be clinically significant because the safety and efficacy studies of oral suspension in pediatric patients were conducted without regard to food intake. Therefore, cefdinir may be taken without regard to food.</paragraph>

                  <content styleCode="italics">Cefdinir Suspension</content>

                <paragraph>In adult subjects, median (range) maximal blister fluid cefdinir concentrations of 0.65 (0.33 to 1.1) and 1.1 (0.49 to 1.9) mcg/mL were observed 4 to 5 hours following administration of 300 and 600 mg doses, respectively. Mean (± SD) blister C<sub>max</sub> and AUC<sub>(0-∞)</sub> values were 48% (± 13) and 91% (± 18) of corresponding plasma values.</paragraph>

                  <content styleCode="italics">Tonsil Tissue</content>
                </paragraph>
                <paragraph>In adult patients undergoing elective tonsillectomy, respective median tonsil tissue cefdinir concentrations 4 hours after administration of single 300 and 600 mg doses were 0.25 (0.22 to 0.46) and 0.36 (0.22 to 0.80) mcg/g. Mean tonsil tissue concentrations were 24% (± 8) of corresponding plasma concentrations.</paragraph>

                    <paragraph>Metabolism and Excretion</paragraph>
                    <paragraph>Cefdinir is not appreciably metabolized. Activity is primarily due to parent drug. Cefdinir is eliminated principally via renal excretion with a mean plasma elimination half-life (t<sub>½</sub>) of 1.7 (± 0.6) hours. In healthy subjects with normal renal function, renal clearance is 2 (± 1) mL/min/kg, and apparent oral clearance is 11.6 (± 6) and 15.5 (± 5.4) mL/min/kg following doses of 300 and 600 mg, respectively. Mean percent of dose recovered unchanged in the urine following 300 and 600 mg doses is 18.4% (± 6.4) and 11.6% (± 4.6), respectively. Cefdinir clearance is reduced in patients with renal dysfunction (see Special Populations, <content styleCode="italics">Patients with Renal Insufficiency</content>).</paragraph>
                    <paragraph>Because renal excretion is the predominant pathway of elimination, dosage should be adjusted in patients with markedly compromised renal function or who are undergoing hemodialysis (see <content styleCode="bold">
                        <linkHtml href="#LINK_7f103881-6664-4693-bdd4-48af40645934">DOSAGE AND ADMINISTRATION</linkHtml>
                      </content>).</paragraph>
                    <paragraph>Special Populations</paragraph>
                    <paragraph>
                      <content styleCode="italics">Patients with Renal Insufficiency</content>
                    </paragraph>
                    <paragraph>Cefdinir pharmacokinetics were investigated in 21 adult subjects with varying degrees of renal function. Decreases in cefdinir elimination rate, apparent oral clearance (CL/F), and renal clearance were approximately proportional to the reduction in creatinine clearance (CL<sub>cr</sub>). As a result, plasma cefdinir concentrations were higher and persisted longer in subjects with renal impairment than in those without renal impairment. In subjects with CL<sub>cr</sub> between 30 and 60 mL/min, C<sub>max </sub>and t<sub>½</sub> increased by approximately 2 fold and AUC by approximately 3 fold. In subjects with CL<sub>cr</sub>&lt; 30 mL/min, C<sub>max</sub> increased by approximately 2 fold, t<sub>½</sub> by approximately 5 fold, and AUC by approximately 6 fold. Dosage adjustment is recommended in patients with markedly compromised renal function (creatinine clearance &lt; 30 mL/min; see <content styleCode="bold">

Dialysis (4 hours duration) removed 63% of cefdinir from the body and reduced apparent elimination t<sub>½</sub> from 16 (± 3.5) to 3.2 (± 1.2) hours.                     </paragraph>
                    <paragraph>The effect of age on cefdinir pharmacokinetics after a single 300 mg dose was evaluated in 32 subjects 19 to 91 years of age. Systemic exposure to cefdinir was substantially increased in older subjects (N = 16), C<sub>max</sub> by 44% and AUC by 86%. This increase was due to a reduction in cefdinir clearance. The apparent volume of distribution was also reduced, thus no appreciable alterations in apparent elimination t<sub>½</sub> were observed (elderly: 2.2 ± 0.6 hours vs young: 1.8 ± 0.4 hours). Since cefdinir clearance has been shown to be primarily related to changes in renal function rather than age, elderly patients do not require dosage adjustment unless they have markedly compromised renal function (creatinine clearance &lt; 30 mL/min, see <content styleCode="italics">Patients with Renal Insufficiency</content>, above).</paragraph>

                <paragraph>As with other cephalosporins, bactericidal activity of cefdinir results from inhibition of cell wall synthesis. Cefdinir is stable in the presence of some, but not all, β-lactamase enzymes. As a result, many organisms resistant to penicillins and some cephalosporins are susceptible to cefdinir.</paragraph>
                <paragraph>Resistance</paragraph>
                <paragraph>Resistance to cefdinir is primarily through hydrolysis by some β-lactamases, alteration of penicillin-binding proteins (PBPs) and decreased permeability. Cefdinir is inactive against most strains of <content styleCode="italics">Enterobacter </content>spp., <content styleCode="italics">Pseudomonas </content>spp., <content styleCode="italics">Enterococcus </content>spp., penicillin-resistant streptococci, and methicillin-resistant staphylococci. β-lactamase negative, ampicillin-resistant (BLNAR) <content styleCode="italics">H. influenzae </content>strains are typically non-susceptible to cefdinir.</paragraph>

IF CEFDINIR IS TO BE GIVEN TO PENICILLIN-SENSITIVE PATIENTS, CAUTION SHOULD BE EXERCISED BECAUSE CROSS-HYPERSENSITIVITY AMONG β-LACTAM ANTIBIOTICS HAS BEEN CLEARLY DOCUMENTED AND MAY OCCUR IN UP TO 10% OF PATIENTS WITH A HISTORY OF PENICILLIN ALLERGY. IF AN ALLERGIC REACTION TO CEFDINIR OCCURS, THE DRUG SHOULD BE DISCONTINUED. SERIOUS ACUTE HYPERSENSITIVITY REACTIONS MAY REQUIRE TREATMENT WITH EPINEPHRINE AND OTHER EMERGENCY MEASURES, INCLUDING OXYGEN, INTRAVENOUS FLUIDS, INTRAVENOUS ANTIHISTAMINES, CORTICOSTEROIDS, PRESSOR AMINES, AND AIRWAY MANAGEMENT, AS CLINICALLY INDICATED.</content>

            <paragraph>
              <content styleCode="italics">Clostridium difficile</content> associated diarrhea (CDAD) has been reported with use of nearly all antibacterial agents, including cefdinir, and may range in severity from mild diarrhea to fatal colitis. Treatment with antibacterial agents alters the normal flora of the colon leading to overgrowth of <content styleCode="italics">C. difficile</content>.</paragraph>
            <paragraph>
              <content styleCode="italics">C. difficile</content> produces toxins A and B which contribute to the development of CDAD. Hypertoxin producing strains of <content styleCode="italics">C. difficile</content> cause increased morbidity and mortality, as these infections can be refractory to antimicrobial therapy and may require colectomy. CDAD must be considered in all patients who present with diarrhea following antibacterial use. Careful medical history is necessary since CDAD has been reported to occur over two months after the administration of antibacterial agents.</paragraph>
            <paragraph>If CDAD is suspected or confirmed, ongoing antibacterial use not directed against <content styleCode="italics">C. difficile</content> may need to be discontinued. Appropriate fluid and electrolyte management, protein supplementation, antibacterial treatment of <content styleCode="italics">C. difficile</content>, and surgical evaluation should be instituted as clinically indicated.</paragraph>

                <paragraph>Prescribing cefdinir for oral suspension in the absence of a proven or strongly suspected bacterial infection or a prophylactic indication is unlikely to provide benefit to the patient and increases the risk of the development of drug-resistant bacteria.</paragraph>
                <paragraph>As with other broad-spectrum antibiotics, prolonged treatment may result in the possible emergence and overgrowth of resistant organisms. Careful observation of the patient is essential. If superinfection occurs during therapy, appropriate alternative therapy should be administered.</paragraph>
                <paragraph>Cefdinir, as with other broad-spectrum antimicrobials (antibiotics), should be prescribed with caution in individuals with a history of colitis.</paragraph>
                <paragraph>In patients with transient or persistent renal insufficiency (creatinine clearance &lt; 30 mL/min), the total daily dose of cefdinir should be reduced because high and prolonged plasma concentrations of cefdinir can result following recommended doses (see <content styleCode="bold">

                <paragraph>Patients should be counseled that antibacterial drugs including cefdinir for oral suspension should only be used to treat bacterial infections. They do not treat viral infections (e.g., the common cold). When cefdinir for oral suspension is prescribed to treat a bacterial infection, patients should be told that although it is common to feel better early in the course of therapy, the medication should be taken exactly as directed. Skipping doses or not completing the full course of therapy may (1) decrease the effectiveness of the immediate treatment and (2) increase the likelihood that bacteria will develop resistance and will not be treatable by cefdinir for oral suspension or other antibacterial drugs in the future.</paragraph>
                <paragraph>Antacids containing magnesium or aluminum interfere with the absorption of cefdinir. If this type of antacid is required during cefdinir for oral suspension therapy, cefdinir for oral suspension should be taken at least 2 hours before or after the antacid.</paragraph>
                <paragraph>Iron supplements, including multivitamins that contain iron, interfere with the absorption of cefdinir. If iron supplements are required during cefdinir for oral suspension therapy, cefdinir for oral suspension should be taken at least 2 hours before or after the supplement.</paragraph>
                <paragraph>Iron-fortified infant formula does not significantly interfere with the absorption of cefdinir. Therefore, cefdinir for oral suspension can be administered with iron-fortified infant formula.</paragraph>
                <paragraph>Diabetic patients and caregivers should be aware that the oral suspension, 250 mg/5 mL contains 1.37 g of sucrose per teaspoon and 125 mg/5 mL contains 1.50 g of sucrose per teaspoon.</paragraph>
                <paragraph>Diarrhea is a common problem caused by antibiotics which usually ends when the antibiotic is discontinued. Sometimes after starting treatment with antibiotics, patients can develop watery and bloody stools (with or without stomach cramps and fever) even as late as two or more months after having taken the last dose of the antibiotic. If this occurs, patients should contact their physician as soon as possible.</paragraph>

If iron supplements are required during cefdinir for oral suspension therapy, cefdinir for oral suspension should be taken at least 2 hours before or after the supplement.</paragraph>
                    <paragraph>The effect of foods highly fortified with elemental iron (primarily iron-fortified breakfast cereals) on cefdinir absorption has not been studied.</paragraph>
                    <paragraph>Concomitantly administered iron-fortified infant formula (2.2 mg elemental iron/6 oz) has no significant effect on cefdinir pharmacokinetics. Therefore, cefdinir for oral suspension can be administered with iron-fortified infant formula.</paragraph>
                    <paragraph>There have been reports of reddish stools in patients receiving cefdinir. In many cases, patients were also receiving iron-containing products. The reddish color is due to the formation of a nonabsorbable complex between cefdinir or its breakdown products and iron in the gastrointestinal tract.</paragraph>

Decreased body weight occurred in rat fetuses at ≥ 100 mg/kg/day, and in rat offspring at ≥ 32 mg/kg/day. No effects were observed on maternal reproductive parameters or offspring survival, development, behavior, or reproductive function.</paragraph>
                    <paragraph>There are, however, no adequate and well-controlled studies in pregnant women. Because animal reproduction studies are not always predictive of human response, this drug should be used during pregnancy only if clearly needed.</paragraph>

                    <paragraph>In clinical trials, 2289 pediatric patients (1783 U.S. and 506 non-U.S.) were treated with the recommended dose of cefdinir suspension (14 mg/kg/day). Most adverse events were mild and self-limiting. No deaths or permanent disabilities were attributed to cefdinir. Forty of 2289 (2%) patients discontinued medication due to adverse events considered by the investigators to be possibly, probably, or definitely associated with cefdinir therapy. Discontinuations were primarily for gastrointestinal disturbances, usually diarrhea. Five of 2289 (0.2%) patients were discontinued due to rash thought related to cefdinir administration.</paragraph>
                    <paragraph>In the U.S., the following adverse events were thought by investigators to be possibly, probably, or definitely related to cefdinir suspension in multiple-dose clinical trials (N = 1783 cefdinir-treated patients):</paragraph>

Hemodialysis removes cefdinir from the body. This may be useful in the event of a serious toxic reaction from overdosage, particularly if renal function is compromised.</paragraph>
